RightSight Health Physicians, PLLC (RightSite Health Physicians) is committed to protecting the privacy and confidentiality of your health information. This Notice of Privacy Practices describes how we may use and disclose your Protected Health Information (PHI), your rights regarding your PHI, and how you can access it.
Your Health Information Rights
You have the right to:
- Access and Obtain a Copy: Request a copy of your medical records and other health information maintained by RightSite Health Physicians. Electronic copies or summaries will typically be provided within 30 days. Paper copies or summaries may incur a reasonable, cost-based fee.
- Request a Correction: Ask us to amend incorrect or incomplete health information. We may decline but will provide a written explanation within 60 days.
- Request Confidential Communications: Specify how or where we communicate with you about your health information. Reasonable requests will be accommodated.
- Restrict Disclosures: Request limitations on how your PHI is used or disclosed for treatment, payment, or operations. Requests may be declined if they could impact your care.
- Receive a List: Request a list of disclosures of your health information for up to six years prior to your request, excluding disclosures for treatment, payment, healthcare operations, or specific exclusions. One free list annually; additional requests may incur a service fee.
- Receive a Paper Copy: Obtain a paper copy of this Notice, even if accessed online.
- Select a Representative: A legal representative (medical power of attorney or guardian) can exercise your rights regarding your PHI, provided they have verified authority.
- File a Complaint: If your privacy rights are violated, file a complaint with RightSite Health Physicians or the U.S. Department of Health and Human Services:

How We Use and Disclose Your Health Information
We may use and disclose PHI for the following purposes without explicit authorization:
- Treatment: To provide, coordinate, and manage your healthcare, including consultations and referrals.
- Payment: To bill and collect payment, including sharing information with insurance companies.
- Healthcare Operations: To improve services, train staff, and ensure quality care.
- As Required by Law: To comply with laws, including reporting abuse or neglect and responding to legal proceedings.
- Public Health and Safety: To support public health initiatives and prevent serious threats.
- Emergency Situations: To ensure timely care during emergencies, even when contacting family or EMS personnel.
Other Uses of Your PHI
For other uses, we will require your written authorization. You may revoke authorization at any time in writing.
How We Protect Your Information
We use administrative, technical, and physical safeguards to protect your information from unauthorized access, use, or disclosure. We do not sell or share your health information for marketing without your written consent.
